Telehealth Appointments
Available by telephone (Canadian area codes only) and through Doxy.me

Ensure your phone is set up to receive calls from anonymous or unknown numbers.

Double check that your computer, tablet or mobile device has a working microphone, camera and a reliable internet connection.

  1. Please check in 5 minutes prior to your scheduled appointment time.
  2. When you are ready, open Chrome, Firefox or Safari on your device.
  3. Go to https://doxy.me/UbiquityHealth.
  4. Type in your full name and click Check In.

Please find a quiet place for your appointment. We cannot conduct the appointment if you are driving or distracted.

In-Person Appointments
Masks are strongly recommended and appreciated. In-person appointments will held at Cross Roads Medical Clinic using shared space. Please note that their staff will be there to greet you but will not have access to your chart and cannot help with administrative tasks such as scheduling. This clinic operates separately from theirs.

Best efforts are made to keep on schedule. At times, we may run 30-60 minutes behind for reasons beyond our control. Thank you for your patience.
